How Do I....
Obtain A Repeat Prescription?

All patients on regular repeat prescriptions will be given a repeat prescription card. Please allow two working days to be sure the prescription is done. If you wish you can enclose a stamped addressed envelope and we will return it to you without you having to attend personally. A box is available at the reception desk in which to place your repeat prescription.

We advise patients to see the doctor every six months for a review of their medication.

THIS FORM BELOW IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
